Telehealth Informed Consent

This consent describes telehealth services provided by the independent, US-licensed medical providers you may be connected with through the LightenMD program. Please read it carefully. By using the clinical services, you consent to receive care via telehealth as described below.
1. What Telehealth Is
Telehealth is the delivery of health care services using electronic communications — such as secure messaging, questionnaires (asynchronous care), phone, or video — when you and your provider are not in the same location. Your provider may use the information you submit, including your health history and intake responses, to evaluate and treat you.
2. Expected Benefits
- Access to licensed medical care without travel or scheduling delays
- Convenient follow-up and ongoing communication with your care team
- Efficient evaluation for treatment programs where clinically appropriate
3. Possible Risks and Limitations
- Telehealth is not appropriate for every condition; your provider may determine you need an in-person examination and refer you accordingly
- Information transmitted may be insufficient (e.g., incomplete history) to allow appropriate medical decision-making; accurate and complete answers are essential
- Delays in evaluation or treatment could occur due to technology failures
- In rare cases, security protocols could fail, causing a breach of privacy of personal health information
4. Alternatives
You may seek in-person care from a provider of your choice at any time instead of, or in addition to, telehealth services.
5. Medications
Whether any medication is appropriate for you is determined solely by the licensed provider evaluating you. If a medication is prescribed, it will be dispensed by a licensed pharmacy and billed separately from your LightenMD program fee. You should report side effects to your provider promptly and seek emergency care for severe reactions.
6. Not for Emergencies
Telehealth services are not for emergencies. If you believe you are experiencing a medical emergency, call 911 immediately.
7. Privacy
Laws that protect the privacy and security of health information apply to telehealth. Providers and pharmacies maintain records of your care as required by law. See our Privacy Policy for details on how information is handled.
8. Your Rights
- You may withhold or withdraw this consent at any time, without affecting your right to future care, by emailing support@lightenmd.com
- You may ask questions about telehealth and your treatment at any time
- You have the right to know the identity and licensure of providers treating you
9. Acknowledgment
By proceeding with the LightenMD program's clinical services, you acknowledge that you have read and understand this consent, you voluntarily agree to receive services via telehealth, and your questions (if any) have been answered.
