HIPAA & Health Information Privacy

This page explains, at a high level, how health information is handled when you use the LightenMD program. It is not a substitute for the formal Notice of Privacy Practices issued by a healthcare provider or pharmacy that is a HIPAA-covered entity.
Who Handles Your Information
LightenMD provides non-clinical technology and program support. Independent licensed medical providers and licensed pharmacies handle clinical records and pharmacy information in connection with the services they provide. Those entities may be subject to the Health Insurance Portability and Accountability Act of 1996 ("HIPAA") and other health-privacy laws and may provide their own Notice of Privacy Practices.
How Health Information May Be Used
Subject to applicable law and the provider's or pharmacy's Notice of Privacy Practices, health information may be used or disclosed for purposes such as:
- Treatment, care coordination, prescribing, and pharmacy fulfillment
- Payment and healthcare operations
- Patient support, safety, quality, and legal or regulatory obligations
- Other purposes that you authorize or that applicable law permits or requires
Your Health-Information Rights
Depending on the entity holding the record and applicable law, you may have rights to:
- Inspect or obtain a copy of your health information
- Request an amendment to information you believe is incorrect or incomplete
- Request an accounting of certain disclosures
- Request restrictions or confidential communications
- Receive a paper or electronic copy of the applicable Notice of Privacy Practices
- Submit a privacy complaint without retaliation

LightenMD and the organizations that support the program use administrative, technical, and physical safeguards designed to protect personal and health information. No electronic system can be guaranteed to be completely secure.
